4 doses of tetanus (1 dose on or after 4th birthday)

4 doses of diphtheria (1 dose on or after 4th birthday)

3 doses of polio

2 doses of measles

2 doses of mumps

1 dose of rubella (German measles)

3 doses of hepatitis B

2 doses of varicella (chickenpox) vaccine or history of disease

Student entering 7th grade must have 1 dose of Tdap within last five years and 1 dose of meningococcal vaccine.

STUDENTS WHO ARE NOT IN COMPLIANCE WITH THE NEW IMMUNIZATION REQUIREMENTS THIS SCHOOL YEAR WILL BE EXCLUDED FROM SCHOOL ON MAY 1, 2012 AS MANDATED BY LAW. Your child's school immunization record is based on documentation you have provided. Please be sure to provide your child's school nurse with any updated immunization information so that his/her records are accurate. If you have any questions please do not hesitate to contact your child's school nurse.

The Chester County Health Department offers free walk-in immunizaton clinics on Tuesdays 9-4, Wednesdays 11-6, and Fridays 9-4.
